We keep a laminated card with all these details in our GO-bag. What's nice about the laminated version is that we can use dry erase markers on it and use it as a checklist too. Ours is hole-punched and tied to the zipper so it can't get lost because it's imperative that the GO-bag is always fully stocked in case of emergency. You can edit the list for your specific needs.
At one point, we were keeping an additional trach suction log because we needed to prove we needed a certain level of nursing care in the home. This log is also nice so you can look back and see if there are certain trends in schedule and suction needs.
Ellie started using a secondary setting on her ventilator to start the (long slow) process of weaning her off her ventilator. There have several bumps in the road but we're still working hard to be vent-free during the day... some day. This is a great log to help track how the weaning process is going. Plus, if you take this with you to your pulmonologist or in our case to Vent Clinic, your health care team will be impressed. ;)
